Advanced hepatocelluar carcinoma (HCC) with portal vein thrombosis has a poor prognosis due to inadequate tumor response. 1 year survival rate and median surviral of stage IVa is less than 20%, 3 months respectively. Transarterial chemoembolization (TACE) was known to prolong the overall survival rate in patients with moderately advanced HCC. But, only small number of patients with stage IVa can receive the TACE. Other treatment modalities to manage for advanced HCC are systemic chemotherapy, hepatic arterial infusion chemotherapy or radiotherapy. Especially concurrent chemo-radiotherapy would be a better treatment option. We report a case of good response after repeated hepatic arterial infusion chemotherapy and radiotherapy for patient in advanced HCC with portal vein thrombosis.
